Anyone can use this plan, regardless of immigration status, although our immigrant families have special concerns that are also addressed in this packet, including knowing your rights and resources in case there is concern about an undocumented member of the family.

Custodial Parent Authorization and Caregiver Affidavits
An important part of your family preparedness plan is choosing someone who could care for your child if you are unable to do so, or in case family members are separated.
For some families it may be appropriate to have a Custodial Parent Authorization as part of a plan you make ahead of time. This form notifies your school that you have authorized another parent to act on your behalf for school matters. You can download the form here: Custodial Parent Authorization
